All it is is a phrase of kindness. I mean, she may genuinely care for your well being, but it doesn't mean anything more than that. Everyone says it to everyone every day. There is no secret message in that.

She cares about you, despite your differences in the past. She is concerned because she believes there is a risk you will come to harm. Simple as that. You already know you do something in excess, respond to the kind words by reducing the risk to as close to zero as possible. And see how much happier she is that you would do this for her :)

It means she doesn't want you to hurt yourself. She's showing that she cares what happens to you, and she might simply do that because she's a good person. Having "problems in the past" doesn't necessarily have any bearing on her not wanting you to get hurt doing something to "excess."

It means that she doesn't like the fact that you drink too much, take too many drugs, work out to much, or whatever it is that you do too much of and doesn't want you to end up dead or coming to harm.
If you've had problems in the past, she either cares because she's a decent person or she is indifferent and is saying 'do what you want'.
