Narcissistic might fill the bill. It suggests someone who is so wrapped up in himself, he doesn't even notice others.
I've linked a page with other choices below. My method was to look for synonyms for selfish, and then scan them for choices that suggest ignorance.

I recall you trying for information on this in the Philosophy section earlier.... here you may get words like pathological or even psychotic; I really don't know. But I do see concern about this, and so I think you should call a professional (it's free) at the closest psychiatric ward to you and ask them what a word may be or anything else concerning you about this. EDIT: Although I like narcissistic for the selfish part, it doesn't cover the "ignorant (unaware of hurting others or the world around someone)..." so I still advise you make the call.

I think the word "narcissist" is a very good choice. I also immediately thought of "ruthless," but I am not sure one can be ruthless and still be unaware of hurting others.

I agree with your first poster. Narcissistic. A person who is the center of their world to the exclusion of others and has no way of relating to the world around them in a healthy way.
