Yes, every time I've had to go to one in over ten years. Because I never wear footwear of any kind and I still have to go sometimes. It's not that gross, my skin is very tough, I never have any cracks or injuries, and our skin is made to keep pathogens out. We're at far greater risk of picking something up through our hands, even if we wash them (think the tap itself, the door knob, etc). It's with my hands I'm much more likely to touch my face or food afterwards; unless I lick my feet or put them up on the dinner table, any dirt I pick up -from a restroom or anywhere else- will just sit there harmlessly.
Parasites aren't a problem, they might still be in some tropical areas but as far as the US & Europe are concerned, the hookworm is the only one that can penetrate the skin of the foot, and it's been pretty much eradicated from the South since modern plumbing has replaced the outhouses, & never was a problem in cooler climates. The odds of finding any even in a restroom are extremely low.
If I were to put on footwear just to go into the restroom, I'd have to *handle* that footwear before and after, and I'd have to carry that dirty footwear somewhere, like in a bag or something, where the fresh dirt on the soles is in contact with anything else I'll be carrying, including possibly snacks and such... so if you're barefoot (even if you normally aren't, think about a park, pool or beach) you're actually increasing the risk of picking something up if you put on footwear just to go to the restroom. Unless you have a foot injury, just walk in and walk out, don't lick your toes and don't worry about it!

Three times at the beach (concrete floors) and once in a store (tile floor). Thankfully the store was clean, because I can't tolerate wet floors even with shoes on, and don't believe at all that urine that has been on the floor for longer is still sterile. The beach didn't matter, however, since any dirt would be cleaned away walking on the sand even before walking along the sand by the water or going into the water.

eww yeah on the coastline and the water park bogs.. reason i grew to become into gettin a tub or washed off, and that i did no longer have shoes to positioned on i won't be in a position to stand it nonetheless. so now i continually positioned on turn flops
