As one answer indicated...for what? What you call a "mushroom cloud" is simply a formation of particles thrust into the air by an explosion of some sort. It need not be an atomic explosion; in fact, many large conventional explosions form mushroom clouds.
The mushroom shape is formed when the hot gases and debris from the explosion rise and "bump" into the cooler upper air. That cooler air saps the rising column of gas and debris of energy, so its climb to altitude slows down. Which means the column of gas underneath runs into the slowed down gas above. That forms the spreading cap of the mushroom.
Asking the average height and width of a mushroom cloud is like asking what is the average energy of explosions. But here are some examples of well known atomic explosions.
"This project was designed to provide information for the solution of two problems: (1) dispersion of gaseous debris; and (2) initial cloud dimensions.^Observations indicated that the major portion of the Mike cloud entered the stratosphere - the cloud top reached 120,000 feet with the mushroom base at about 67,000 feet.^The tropopause height at the time of the detonation was about 58,000 feet.^The top of the King cloud reached about 74,000 feet with the mushroom base at about 40,000 feet" [See source.]

The cloud goes into the stratospher so your're talking at least 15-25 km high.

For what? A couple of sticks of dynomite? A hundred feet maybe. A couple kiloton nuclear bomb? A mile wide maybe. A couple megaton nuclear bomb? a few ten's of thousands of feet high, a couple miles wide.
