I recommend the Oregon Bookbinding Co. They have done excellent work for me. I mail them manuscripts, and they mail back the bound books.

Cafepress.com is a GREAT way to make your document into a hard-cover book. Its free, too! And there arent any middlemen or publishing companies to push you around. You design the book yourself and it's instantly up for sale to anyone! Its cool, check it out!
You could also design T-shirts to go along with your book! ;)

Check with a print shop. Or a bookbinding shop. There are fewer of those around now, but maybe in the bigger cities. Office Max or Office Depot do those plastic bindings. I forget what they are called.
