Something nastry in the food, or I just plain didn't like it. It all comes down to the Chef. But I would most likely take this up with the manager.

I have found hair in my food not on the food actually cooked into the meat and yes I complained to the manager on duty and later to the owner of the restaurant after the manager was very rude to me and accused me of placing the hair there myself

If I found something nasty I would tell the server and see if I could get a replacement.
If I didn't like the food, I wouldn't say anything but I would never return to eat there.

I would think anyone from the prep cook to the cook might have been working on the meal when it happened, but as a manager EVERYTHING that happens in your restaurant is your responsibility. The owner, not so much unless they were the cook. It always falls on the manager, been there done that.

It is most likely to be the chefs fault
then again having worked in restaurants all sorts of things happen behind the scenes
it is always wise never to be rude/obnoxious to serving staff

I've found many things in my food, many times. Mostly I call the waiter and show it to them. If it is a fly, they take it away and give it a decent burial. I continue with my meal.
