Brainmapping. Enough research has been done now so that a neuraldevelopmental psychologist can "see" from a EEG of the brain if the patterns are typical of A.D.D.

Beware of surveys and opinions. ADD is way over diagnosed!

You want to get a neuropsych evaluation. Can be done by either a psychologist or a psychiatrist who does evals on kids.
They'll run a standard battery of tests. If your child doesn't show any sort of emotional problems then they may switch those tests out for others. It will probably include the WISC (kid's IQ test), some motor activities to gauge attention, and some general talk evaluations.
Be ready to do some evaluating yourself with the materials that they give you. Depending on the child's age you may be giving a set to certain of his/her teachers as well.
Get a copy of Driven to Distraction and Delivered from Distraction by Hallowell. He covers a variety of different approaches to diagnosis, treatment, etc etc.
There is one thing though: you definitely want to get the diagnose before you start to work on the problem.
Don't be surprised if during the process either you or your spouse are diagnosed with ADD as well.
Good luck.

My son's neurologist sat and talked with my son for a while, observing his behavior and impulses. He then gave me some paperwork to complete and have completed by his teachers with questions on his behaviors, sort of like a check list with room for details. He then observed him again during the second appointment as well as read the feedback from me and my son's teachers. My son also has a tic disorder, which many times accompanies ADD. After a few appointments he was diagnosed with ADD and it is highly likely that I have it as well.

They send you to a psychologist who then gives you a series of tests that you have to complete. I went with my friend when she got tested and it took about 3 hours total.
