Most people waste a lot of time and money trying to remove mold / mildew from their shower curtains, They think bleach is the answer to all cleaning . Although using bleach seems to work , unfortunately it only last for a month and you have the same problem. I lost count of the shower curtains I bought, So got fed and fitted shower doors easier to clean and save´s a lot of money. You can kill of mildew / mold by exposing a article of clothing to the sun and by adding lemon juice and leaving to dry , before washing as normal. Also the same for dry cleaning fabrics , except you add lemon juice and salt expose to the sun , then wipe down with a wet sponge

You probably can just get away with spraying a mildew remover on it and then rinsing it off. Look in the cleaning products section at the grocery store for a spray-on mold and mildew remover (a few companies make it). Spray the stuff on both sides, making sure to get between the pleats. Leave it set for a while (about 15 minutes) and see if the mildew is gone. If it's not completely gone, reapply the spray. Then rinse it off.
Also, do not breathe in the fumes. The sprays contain chlorine, which can be very harmful to sinus tissue and lungs.

I agree with a lot of the people here - bleach in the washer will do it (I *do* put mine in the dryer if it's cloth but not if it's vinyl), and mildew remover from a store/rinsing/reapplying works on vinyl, but wanted to add that vinegar and sunlight also work in case you don't want to bleach out the curtain (1/2 cup vinegar in washing machine, soak, put out in sunlight for a couple hours, finish wash cycle as normal, rewash with a cup of vinegar if first time doesn't get it).
Good luck! (:

1] wash in hot water with a few towels [ cruddy ones - they help scrape the mildew off ], soap, and a bit of bleach
2] put in dryer for five minutes, then hang up to dry
3] cut off bottom hem of plastic shower curtain - the water runs off, taking most of the 'would-be-mildew' with it.
