It does vary by state. The minimum amount of education is a Bachelor's degree. Many states require a Masters Degree as well.

Most degrees specialize a certain kind of education. For instance, a BA in Elementary Education is a little different than a BA in Secondary Education. There is also Special Education to consider as well.

Some states also require that you take a certain amount of credit hours in a certain subject. For instance to be a high school Chemistry teacher you need to get a BA in Secondary Education and have 18 hours of college chemistry classes.

All states require for teachers to become certified by the state education board. the requirements for that vary as well.

There isn't a specific program. Each State has its own rules but generally you can start teaching with a Bachelors Degree and temporary teacher certification but need a Masters for permanent certification. Check with the State Ed. department in the state that interests you.

You will need a college degree with a major or minor in English. Other requirements vary by state. You will have to contact a college education department and see what the requirements are for your state.
