Bread always changes, i like sourdough, wheat, dutch crunch.
Meat - turkey, salami or ham
Cheese - normally choice between chedder and provologne
Condiments - lettuce, tomato and pickles

I am so seriously into Miracle Whip, Spicy Mustard and processed cheese. Also tomatoes, and a little lettuce. This is pretty much with any kind of meat.
