It depends on what you're doing in the water. If you were forcefully submerged with no equipment and vomited water would of course enter your airway if you did not hold it in resulting in drowning unless you were able to surface and breathe again. However, most people underwater who felt the need to vomit would immediately surface and do so. So long as you stayed afloat this would not be an issue.

However, if you were scuba diving with a regulator you would be able to force the substance out and while you may choke briefly and get water into your mouth, cycling air back in and water out would be very possible. Many divers have vomited underwater especially in extreme deep dives with long periods of decompression. While it's not fun, it's not fatal if handled properly either.

Your first reflex after vomitting is to inhale. The vomitting itself is not the dangerous part, but the reflex afterwards.

Without gear or a means to stand up (in shallow water), you will most likely aspire some water. Depending on your depth and what you are doing will depict how severe it becomes.

While scuba diving it is 100% possible to vomit through the regulator. I won't say the regulators are designed to have vomit pass through them, but most of them can handle it. I have vomitted (due to contaminated air) at 55 feet underwater. I held the regulator in my mouth and just puked away as I swam to the surface. In between vomitting sessions, I would rinse my regulator and admire all the fish that showed up.

it depends on how you are swimming, and i think it is rather good to vomit under water. instead if you want to vomit you come out side of the swiming pool and vomit.

If you are a few feet under puke as you head to the surface and live... if you are deep I hope your will is current and your family ready for the funeral
