Call your pediatrician and have him/her send you another copy. They don't give you the original, that stays at the doctor's office just for situations like this. Good luck!!

Call your pediatrician and request a new certified copy and remember to ask about the fee usually it is a nominal amount. Once this record is obtained make a copy of it and keep the original in a safe place and give the copy to your mom or another person that you can trust to store for safe keeping just in case.

If you want the official "state booklet" I think you can pick those us at the health department office.
Then just ask your child's pediatrician (or one of the nurses) to fill it out for you the next time you're in the office. They should have all of the same records in your child's chart.

your baby's doctor or the local health department should have the records. they have to keep records of the vaccines they give.

Your pediatrician's nurse can get you a copy. I've had to call her & ask her to fax it to the daycare twice; she's always happy to do so.

contact his dr, they can make a new one for you, It may cost $10 or so. If you have been doing shots at the county or a cilnic they will have records too.
