1. First off, make sure it is tuned up. This is the most important thing to do.
2. Make sure tires are inflated to the proper specs.
3. Don't drive around with extra weight in the car. (My girlfriend lugs around hundreds of pounds of misc junk in her car and the extra weight takes away from your mileage.)
4. Join a car pool.
5. Change your oil, tranny oil and rear end oil to synthetic. Your motor will run more efficient and your rolling friction will be reduced using synthetic oil. (Most effective in cold climates.)
6. Use blended fuels like Ethanol which is made from corn or other bio materials.
7. Plan your week so you make as few trips as possible.
8. Change your driving habits to avoid rapid starts and stops. Reduce your speed.
9. Warm up your car just enough. When the idle drops, it's ready to drive. Don't leave it running for 10 minutes while you run around looking for your coat.
10. Ride your bike to work!

The biggest change you can probably make is to learn how to drive efficiently - take an economy driving course. Most drivers use up to 20% more fuel than necessary just by having a driving style that is not the most efficient.
The simplest change you can make to your car is to fit tyres with a lower-rolling resistance (e.g. Michelin Energy), and to ensure that the tyres are always at the car manufacturer's recommended pressure (or maybe even 1-2psi higher). However, do not change your tyres until the existing tyres are worn, or you'll have to factor the cost & environmental issues of disposing of perfectly usable tyres.
Also, do not carry unnecessary weight in your car. Remove heavy items from the boot when not needed (e.g. golf clubs). Don't have a roof rack on the car if you're not carrying something vital on it.

Don't brake hard - unless in an emergency;
don't use an automatic car - use a manual;
maintain your tyres at the correct pressure for the trip;
Drive with as little extra weight in the car as possible;
Do keep your car 'maintained' to be as fuel efficient as possible;
Don't wash your car unless necessary - water from a water butt is as good as tap!
Don't run your engine unnecessarily at railway gates - waiting for a train.
