I know there are different schools of thought on this, but I'm going to say: when your brain and heart together are no longer functioning. Machines can keep a body viable, which is to say it can make you breathe and make your heart beat.
But when your brain activity is gone and your heart stops beating (whether you are on a ventilator or not), you are dead.

Death is uncertain. It may come uninvited, at any time. Generally a person dies when its inner soul, i.e. conscience is dead & does not happen to be for the wellbeing of any other.
