I'm currently at a community college in Illinois and was planning to go to the University of Illinois at Chicago next fall. If I get a 4.0 am I likely to be accepted, along with the minimum amount of credit hours? I think it's 24 when applying and 36 when transferring? Is that correct?

You are considered a transfer student if you have successfully completed 24 college hours. If you have less than 24 hours, you are just considered a new freshman.
Here's a listing of each college's transfer requirements: http://www.uic.edu/depts/oar/undergrad/transfer_requirements_undergrad.html
