I'll start the list with two:

1. The Black Crowes - Amorica....there is visible pubic hair on the design.

2. John Lennon - Unfinished Music No.1: Two Virgins....John and Yoko are nude on the cover.

There's also the original cover of "Beatles '65", and the European version of Jimi Hendrix's "Electric Ladyland", and let's not forget UFO's original version of "Force It", or "Blind Faith", the Rolling Stones "Sticky Fingers". "Nutz". Montrose "Jump On It". Pioneers "Feel The Rhythm". Supertramp "Indelibly Stamped". Kin Ping Meh "Virtues & Sins". Roxy Music "Country Life". Frank Zappa "Joe's Garage". Note, that some of these are Euopean versions. Great subject -- got any more to add?

lol I remember the Black Crowes' cover...but I think they only sell the non pubic hair version now.

How about:

NOFX - Heavy Petting Zoo (It's a cartoon of a guy 69ing a sheep)

Dead Kennedys - Frankenchrist (not the actual album cover, but the artwork inside the record contained an illustration by H.R. commonly known as "Penis Landscape." The band was actually criminally charged with conduct harmful to minors, and singer Jello Biafra's house was raided by government agents. He and one other person actually went to trial because of it (the case was eventually dismissed).

Edit: an album by The Dwarves were mentioned above me... actually, pretty much all albums by The Dwarves are controversial.

I think there was a little controversy over Porcupine Tree's Signify cover, the one with the woman being hung in the air by strings. I think alot of women in a women's right group were offended or something.
Not like alot of people knew about it though...that was when PT was still really underground.

The beatles - "Yesterday & Today" - showed the four Beatles in butcher outfits with peices of meat and decapitated dolls all over them

The Rolling Stones "Sticky Fingers" had a picture of a man's crotch, unheard of cover art for the early 70's - doesn't seem so bad today, he was wearing jeans.

well nirvana's "nevermind" has a nude baby boy swimming and that caused a huge protest. a bunch of stores had to put stickers on the front or refused to sell it... i don't think it should have been that big of an issue though.

also marilyn manson's album "mechanical animals" has a pretty werid cover that caused some stir.
