It means that the originally planned outcome of a thing has somehow been changed, and now....through a simple twist of fate...everything is changed.

Imagine that fate is like a road leading to your future. You are meant to travel it and can see what you're going. A twist would suggest something unexpected that could not have been foreseen. It is figurative use of language as you can't really have a twist in fate!

It's an ironic, often unexpected outcome or ending, where destiny overrules an expected outcome. It's a plot twist in one's life, or a charactor in a novel, where the gods (Fates) intervene to create predestined or ordained events not expected, nor forseen by mortals.

My impression is that it is a phrase that means an unexpected and unforeseen turn of events. In other words something happens that one did not plan on.I think the concept of fate itself is a sort of master plan view of life. If we look at life as pre-destined we interpret things as being fate. Fate always fits the plan. When things happen that are not foreseen, in hindsight they fit the plan nonetheless, thus they are referred to as a twist of fate.

It means that an occurrence which seemed unlikely to happen happened, as though the only reason it could have happened is for fate to have intervened.
