The origin is from Shakespeare's "Othello".

Iago (a sneaky, cunning, conniving fellow) says

"For when my outward action doth demonstrate
The native act and figure of my heart
In compliment extern, 'tis not long after
But I will wear my heart upon my sleeve
For daws to peck at: I am not what I am"

He is saying that he's a fake and a fraud, and were his actions to mirror his true thoughts, he would be wearing his heart on his sleeve where it is vulnerable.

It's really not about emotions, it's about one's true nature.

The popular expression of 'Wearing one's heart on one's sleeve' comes from the tradition prevalent in America and Britain around 1800s. At that time, young men used to wear slips of paper pinned on their sleeves and had their girlfriend's name written on it.

Today it means
Show emotions or feelings openly.

If you wear your heart on your sleeve, you openly express your feelings or emotions to others.

Well I literally do, because I have a tattoo of a heart on my wrist (for the very reason of what the phrase means) To wear your heart on your sleeve means that you show your emotions. When you are sad (happy, mad, etc), people can see it.

If you are angry or sad then it shows on your face and in your body language - just like wearing your emotions on a sleeve.
