yeahhh this probably sounds stupid.
BUT
when you insert a tampon is ANY of the plastic applicator supposed to stay inside you or is it just the cotton that stays in...?

The entire plastic applicator is to be removed after the insertion of the tampon. Only the material portion is to remain inside the vagina.

You are supposed to remove the applicator after the tampon is inside you. Do not leave it in, it will hurt.
