It is anything that takes a more serious approach to a cure or treatment. Usually it is for diseases that will take a long time to cure, if they can be cured at all. They also refer to diseases that requires medical attention frequently and medication daily. Finally it is also referring to the possibility of fatality caused from the condition, not so much that one will die in 2 years, but in that it can have an impact on your lifespan.

A serious medical condition means the person is unable to function normally and needs medical attention from a doctor. It can be temporary (the person recovers) or debilitating or may last for a while before he or she recovers.
