Hi Carl,
The only way that I am aware of to do this is to open the word document, click on the image that you wish to save. Then hit alt+prnt Scrn on your computer. This saves the image to your clipboard.
At this point, open any photo-editing software that you have. If you do not have photo editing software open your Paint program. Click Edit then paste.
You can save your image from there.
To my knowledge Microsoft Word does not give any options to save an image as a jpeg or gif or any other image formats.

try selecting the "design." press ctrl+c, or right-click then copy.. open paint, or any other image-editing programs.. then press ctrl+v, or right-click on the canvass then paste.. then save the image...

simple trick
display exactly what you want to save as a picture
press print screen button
open paint
go to edit and press paste
job done

actually ur quetion is not clear
i think u want to convert ur design in word document to an image format
for this u can copy it and paste into paint and save in image format(.bmp or .jpeg etc)
or if it doesnt work then use print screen button of ur keyboard
steps
1 get the image in desired size so that it fully comes in the screen of ur computer
2 press print screen button
3 open paint and paste (i.e press ctrl+v)
4 adjust ur image as u want . cut the window borders etc.
5 save ur image with desired name.

in the word menu click insert-->picture-->from file and select the image.

Go to the menu Bar and select INSERT Picture from a File and it will put it right on there.

or ....you can select all objects copy and paste them into powerpoint....in powerpoint you select them all (hold shift down and select with mouse) and right click and select save as picture
