Usually... but there are some exceptions...
The biggest one is when it it pouring outside... the overhang above my window is sub-par and I also have windows that go all the way to the floor... which means if I leave it open too much when it's raining everything gets wet... and if I leave it even slightly cracked when it's pouring... same result.
Another time is when it gets too damn cold outside... When my snot starts to make icicles I tend to start closing the window... =)
There are a few other times... like when the dog-woods bloom (too much pollen)...
I'd say that I leave the window open about 70-80% of the time.

Well, the answer is, rain, sleet or snow the window AND door is open most of the time because I have a phobia about breathing used air.

yes i do. It's good to keep your window open so that the air will be fresh as the circulation process take part. But please take note if your area are plenty of mosquitoes you're not advice to open your window without any barrier that can prevent the mosquitoes to enter your place. It can be dangerous because it can causes dengue.

I live in front of a street so no. The traffic noise would bother me for sleeping. But I do sleep with a fan on.
