Starters or reserves - it doesn't matter. All quarterbacks have the green dot, and nobody else. I don't remember seeing this before this year, but I couldn't say for sure it wasn't there before.

The green dot indicates there is radio communications in the helment and only one player can have that at a time. I think there was suspicion the helmet was being used by other or more players. This way the officials can see easily.

The green dot is usually worn by the QB. Because it has a transmitter in it to help the QB get the play.

It denotes which helmets have earpieces and microphones in them. Only one per team are allowed on the field at a time.
