No. For instance, The Hound of the Baskervilles takes place on the Baskerville Estate on the moors in Dartmoor in Devonshire. Some part of the action takes place in London, and the new owner of the estate has just come over from Canada. Other stories follow Holmes as he solves mysteries at various places outside of London, too.
In The Boscombe Valley Mystery, Holmes goes to Herefordshire. In the Adventure of the Speckled Band, he goes to Stoke Moran in Surrey. In the Adventure of the Final Problem, where Holmes and Moriarty have their epic conflict, Holmes goes to Canterbury, Newhaven and finally to Switzerland (where the meeting takes place). The list goes on and on.

Above answers are correct. *Many* of the adventures take place outside of London (though in England). Many of them have country (rural) settings. They are well worth the read, and you should be able to find them all in PDF online.
