I did tonight, I took a lady down to the hospital and when I brought her back, I forgot some necessary equipment. I was quite ready to go back and get it at the end of the shift but the senior/deputy manager wouldn't let me because it meant a taxi there and a taxi back ... just over a tenner.

I was going to pay for it myself as it was my mistake but she still refused to let me.

well i have not made any huge blunders, because i am a nurse so everything is checked and double checked where medication is involved etc,but i have made mistakes at work like the time i was talking to a patient about the hip operation she was going to have...i sat for 15 minutes explaining what was going to happen and at the end i asked her if she had any questions and she said "so when am i going to have my shoulder operation then"i had picked the wrong notes up but what worried me most was that she wasn't a confused person she didn't stop me and put me right at the beginning she just excepted what i said.but don't worry we wouldn't have operated on her hip we go through rigorously, the information before they actually go to theatre.

Hey, we're not all perfect.
Obviously you can't forget critical things but we're all allowed to slip up every now and again.
If I **** up, I'll try to recover and work around it but I'll be the first to own up.
The worst thing is to hide things or not tell people.

I dont make mistakes in work. If I make a mistake it could kill someone. I am a pharmacy dispenser, so its vital I dont make mistakes.

At home, I make mistakes all the time, but not in work.

I'm retired, so I don't make blunders at work.
I do make blunders in my life though.
