That's the patriotic tradition. In military circles, uniformed persons stand at attention, and un-uniformed persons (spouses, etc.) stand with hand over heart.

During rendition of the national anthem when the flag is displayed, all present except those in uniform should stand at attention facing the flag with the right hand over the heart. Men not in uniform should remove their headdress with their right hand and hold it at the left shoulder, the hand being over the heart. Persons in uniform should render the military salute at the first note of the anthem and retain this position until the last note. When the flag is not displayed, those present should face toward the music and act in the same manner they would if the flag were displayed there.

It's a big no to not to place your right hand on your left chest when there's a flag rising. It's punishable as written in the law. In fact in every law of every nation.

There's no reason to, really. The heart's just the honcho of your circulatory system, it's just that it looks better if a person's putting their hand over the leftish part of their chest rather than over their brain, ya know.

Yes, it is considered proper, they should stand as well. People don't do it for a variety of reasons.
They may be protesting something as we have freedom of expression here.
Perhaps they are not American, they may just be visiting.
Who knows... If it's not those 2 reasons, I'm sure they have their own reason.
