You definitely need a visa to visit India. You could apply to the Indian Embassy in Washington D.C or any of the Consulate offices. Choose the closest office for processing your visa. Here is a list
http://www.immihelp.com/directory/indianconsulatesusa.html
An important note:
- allow yourself sufficient time for visa processing. A few weeks more than what they say.
- don't believe someone who says that a visa can be granted at the Indian airport. This is just asking for trouble.

You need valid passport with visitor visa to visit India. Indian embassy in your country will help you or else there may be some authorised travel agents/ agencies who charge visa fee and arrange visa for you. There is no other special requirement to travel abroad (india)
