Yeah, it's ambiguous. Does the young woman have heart disease, or does her baby?

If the baby has heart disease: The girl whose baby has heart disease is American. Or, The girl, whose baby has heart disease, is American.

If she has it: The girl with a baby and heart disease is American.

This is truly ambiguous since either of them could have the heart disease.

However this is a better sentence.

Notice I also capitalized "American". It is a proper noun.

yes it is. Maybe u can rephrase the sentence to " The American girl with the baby has a heart disease." This will mean that the girl has heart disease-clear and straight to the point.

or
