In college I have a tape of me trying to get on the other team's bench after getting my bell rung so bad that I had no idea where I was.
I got a pretty sour reception at their bench and was promptly dumped back onto the ice as I tried to get over.
It was more than a little sad to say the least and way too embarrassing for me to put on youtube. In fact, I think I will go burn it now.

Once as a ref, i didn't notice the two teams had lined up on the wrong sides and I went ahead and dropped the puck (it was a novice game). The one team won the draw clean and a player ended up scoring on a breakaway on his own net. I had to get help to sort that one out. Man I was so embarassed. I took a lot of abuse (verbal and physical) after the game as well (I was 18) and I decided I was not officiating kids games anymore not long after.

i was a defensemen and i got a pass out on the blue line to take a shot on net. I used to shoot high alot, but this time i shot it so high it went over the glass. Probably cleared it by about 5 feet. I didnt hit anyones stick or anything, i just shot it that high myself.
Another time when i was in peewees or bantoms, i dumped the puck in the zone just outside the blue line with a slap shot. I purposely shot it in the corner, but everyone thought i was trying to shoot it on net. Everyone made fun of me for having horrible accuracy after that.
Also in a big game in high school, i was put on the ice as the extra player as we pulled the goalie to tie up the game in the last 20 seconds or so. We had a face off in the opposing team's zone and i was lined up as the second right winger during the face off. I was right in front of the net and when the puck dropped, our center hit it over to me and i have a wide open net. The goalie dove over to save it and all i had to do was flick it up high in the net. Since i was always shooting high, this was going to be easy for me. But i shot it right in his glove! I missed a wide open net to tie up the game. We had about 400 students from my school watching the game. Very embarrasing! After i shot the puck, i remember getting drilled by some kid. Made the whole situation much worse.
I think i went home and cried myself to sleep after that!

I have two I regret;
1-Falling on a player from the other team while chasing a puck in the corner.I actually broke his ankle and felt terrible.I mean what do you say? Sorry.Felt really bad.
2-The other was small but I was playing D and had a beautiful pass given to me and totally missed the puck and everybody had to clear while I had to chase it down.

Oh this could be fun!
one of my more embarrasing moments would be when I was about 16, we were about to go on the ice for our first face off of the game and Im just kinda gliding out to my positionand BAM the blue line jumped out of no ware and tripped me! That was great.
Another one would have to of been when I was just skating around before warmups not watching where I was going, might have been talking and smack ug the net!!!

probably the time i sent the puck wide when i had a clear shot for the open part of the net. That would be the 3rd and final missed oppertunity for me to score that game and ultimately lead to us losing while i am siting in the box serving somebody elses 2 minutes after they fought

I showed up to the rink and no one from my team was there. Normally our coach would be in but we were the first game of the day and there were only two other people there. I asked what team they were from and they told me a team that wasn't in our league.
I raced over to the other rink about 20 minutes away and got there in time to find out my game was at the first rink and the other guys had lied to me. Of course there is no back-up goalie in adult leagues and I left my team hanging. I got there in time to play about half the game. I felt like a complete ***.

I was clearing the puck in the neutral zone has I passed the puck up I slipped and fell and the opposing team passed me and had a open chance to score with a open net. In which they did and won the game.

I think it had to be the one game where my defenseman(i'm a goalie) came out from behind the net, blew a tire and performed a spinarama at the same time, putting it into our net because I was too busy watching this display and laughing.

1) My first figth in my first training camp, 1 punch TKO and I didn't win.
2) 30+ USA Hockey Nationals. The belt on my shell came loose during play causing it to drop to my ankles. Wouldn't have been all bad except that the Phoenix Coyotes team photographer was on our bench at the time to document it.
