I haven't played it myself, but I've heard good things about Medieval - Total War for the PC. Here is an IGN link with rating (they gave it a 8.9/10)

I personally like the game "Age of Empires"... very realistic medieval war game.
Also, "Rome: Total War" is very realistic.
Both are pc only games.
