1] abnormal bleeding [ placents previa, placental abruption, or unknown ] - an uncontrolled hemorrhage will make it necessary to do an immediate Csection
2] if baby is not head down, or not head down and facing down, they will not fit properly through the birth canal, and could be injured without Csection.[ sometimes the babies who are not face down will still fit, but you have to see if they make progress while pushing to decide.]
3] infection - an outbreak of herpes, for instance, requires a quick Csection to prevent spread to the baby
4] fetal distress - if baby's heart rate shows signs of stress, and then distress [ not responding as should ] then a Csection is required
5] cephalo-pelvic disproportion [ CPD ] - the baby is too big to fit through Mom's pelvis, and mom has a contracted pelvis [ deformed from disease or injury ].
6] maternal disease - if Mom has a heart disease, diabetes, or develops pre-eclampsia, for instance, the doctor's will deliver the baby earlier, and with no stress of contractions
7] fetal disease - open tube defects, oomphalocele, etc, - where baby has organs or tissues exposed outside it's body, which would be injured going through the birth canal.

Only when mother or baby will die or be seriously injured otherwise. It's very, very, very rare for a baby to not "fit" through a mother's pelvis and almost always occurs due to poorly managed gestational diabetes. A breech baby isn't actually a cause for c-section, but not many providers will deliver a breech baby vaginally, although 30 years ago it was safely done all the time. It's a shame so many women don't understand what really warrants a c-section. Read the book Pushed by Jennifer Block for statistics and info about why c-section is way overused in the US.

When you are physically unable to deliver the child naturally.
Sometimes the mother's health is in danger or the child is "in distress", and the baby needs to be delivered quickly to save their lives.

if the baby is breech or the head is bigger than your pelvic bines so it cna't fit. my friend jsut needed one because the baby's head was trying to come out sidewasy and 2 1/2 hours of pushing did nothing but bruise his weet little head
