If its a chain drive, possible the teeth on the sprockets are worn down. If its a screw drive, then the lift attachment could be stripped.

Check the tracks that the door sets on. It's probably misaligned or off by a long shot. If it gets daily use then you may need to check the motor. Helping it by hand also puts extra strain on the motor. So like I said above try replacing the motor.

Check the tracks, they may be working loose, also, check the opener itself. Is it a worm gear (screwdrive, or a track drive) A screw drive may need lubrication. Check the opener assembly itself to see that is is in line with the door tracks.

clutch slipping on drive unit, rollers hanging up, on tracks, is the drive unit belt driven, belt could be slipping, a can of belt dressing at a hardware will help,

Tighten the bolts, and make sure the rollers are not binding on the tracks.
