You know, in all my experiences at a checkout line, not once have I ever gotten close enough to the person checking me out to notice whether their breath smells bad or not....what are you doing, sitting right on the cash register? Have you ever considered the fact that you may be incredibly rude by invading her personal space?
I'd have to say yes, it would be very tacky of you to report her if you don't have the sense enough to back off of another person's space even though you find it so "offensive". Just go to a different line, or a different store, even, but don't go messing with someone else's job and livelihood because you have a problem with respecting other people's boundaries.

"Report" would be a bit harsh. But a mention to the manager of the store would be quite in order - as likely other people may have said the same thing. Many people mentioning it will give weight to the need for the manager to do something about it.

Yes, it would be tacky. Offer her a breath mint...casually. Like, pull your breath mints out of your bag or pocket when you go to pay, take one for yourself, and offer her one.
Halitosis can actually be a sign of gum disease. If you know her pretty well, suggest to her that she go see a dentist.

Well it may seem tacky, however, sometimes you have to do what you have to do. When you speak to her superiors the motive should not be to cause her to loose her job but so that she may be persuaded to take better oral hygiene!!!

honestly, I think it's a hygiene issue, so I would report the cashier! the person can use breath mints or strips!

Talk to her manager. If it's as bad as you say, she needs to be told so she can correct the situation. She could be driving customers to other stores because they don't want to get involved.

You could and that would be fair and reasonable to do but I would buy a small mouthwash and when she goes to ring it up, stop her and say to her "NO, that's for you dear, I don't need it and I have noticed you do". If she gives you any bit of a hard time about it then report her and her stinky breath.
