Holding a Worst Tsar of Russia Contest would be like a Worst American President Contest - - - - -crowded with candidates and no clear conscenus..
The Terrible in Ivan's Name meant he had bad bathing habits, a terrible temper, was mean & capable of killing without provocation but otherwise he was good capable ruler and chiefly due to his ruthlessness Russia achieved a level of greatness.
I'll second that vote for Tsar Nicolas the 2nd - - - but some would argue for another of the Ivan's who was esentially an epileptic blind crippled mute (not that there is anything wong with that), and Catherine's Hubby, Peter the Less than Great the 3 rd is another good choice...

The word translated as 'Terrible' is Grozny, which means impressive in a rather menacing way.
The Russians seem to like that sort of stuff. You certainly don't pick up any brownie points by being a softy there.
Stalin was just such another.

Yes, since people suffered under his rule more then under the rule of any other Tsar of Russia. And the weird thing was that, like other murderers, he was "devoted to God"...

Since that's an opinion and not a history question, I'll say
Tsar Nicholar II of Russia, since he failed to continue his position and allowed it to be tainted by Rasputin.
