You will have no problems in any of Europes cities, or large towns.
If you can't find a public toilet when needed a word with staff in a local pub/restaurant will sort you out no problem.
Also, many large stores now have toilets in them, also all main bus stations - train stations also have them, but only really large ones have them before you get on the platforms - for which you need tickets.

As said France, specially the bigger towns are good, smaller towns all have public conveniences too, mostly easy to find.
In Paris go to the ones just across the river from the Eiffel tower, outside Trocadero, at the entrance to the metro there, beautiful art noveau with nice wood and leaded glass.
England, no problems, all towns and all bigger supermarkets have toilets you can use, well signposted for the public ones, free to use too.
The Netherlands, not so good, you have to rely on cafes and big shops, I find it very hard to do relaxed shopping on the way from work to home.
I often need toilets too, (not often enough to need medical help, but I know your needs,) and I never found real difficulties except in my home country, the Netherlands.

I am in complete sympathy with your plight.
Paris (which once was not a very good place for one in this situation) is now much improved since the city installed a large number of those self cleaning toilets facilities. Better still, the last time I was there they had eliminated the fee for using them.

I can't speak to the bathroom problem directly, but a solution, if the problem is bladder-related, and the patient male, is the good 'ol pilots buddy-bag. The kit comes with a bag that attaches to the lower leg, and a tube from it attached to a condom catheter. While obviously not the PREFERED option, it beats having one's 16-letter bladder alarm getting to O-and-a-half, and no restroom available. (That's letters A thru P, and if you get to "Q", that translatres "Quick get me some clean pants) You wear it as tho you know you'll have to use it, and if you DON'T, it's easy to do what is needed to use the urinal.
