That could happen for a number of reasons - I'd start with a check of the water parameters (ammonia and nitrite = 0, nitrates below 20 [even lower is better], pH 8.2-8.4, salinity 1.024-1.026, temperature below 84).

Depending on the type of starfish, it may not have been getting enough food.

What tankmates do you have for it? If you have fish that aren't reef safe, they may have killed it.

have you tested your saltwater lately ? Maybe your chemicals are not in balance
