Dog tags are normally aluminum sheet that is stamped in the case of those worn by soldiers and engraved on those by dogs.
If you wanted to make an odd or oversized one, you could melt aluminum in a crude foundry and pour it into a dry plaster mold carved or cast from a clay model.
