Actually that dress up stuff sort of gets on our nerves because women (girls) will ask us a bunch of questions we don't know the answers to or couldn't care less about. For example:"Does this dress make me look fat?" That's the beginning of a verbal minefield for a guy who knows he's going to be jumping through hoops all day....we hate that. So, in this case, the carefree fun, crazy girl will win out in the attractiveness department, but there's more to attractiveness than the parameters you gave us.

No concrete answer for this. Guys like different types of girls. I've seen guys with girls that I wouldn't take to a dog show in fear she would be chosen Best in Class. And I've seen ugly guys with such hotties I drool. Go figure.
I don't think you have to really dress up, but wear something decent and makes you look attractive. Remember you are the nectar and we are the bees. Here us humming?

Physically:
thin, firm, healthy ladies are what I go for.
Personality-wise:
Intelligence, confidence, humor, faithful, fun to be around, easy-going.
As far as appearance.....clothing is kind of general. I've seen women that were just drop-dead gorgeous that had jeans and a simple top on....and they look fantastic. Other might wear short-shorts.....short skirt....which I love cuz I'm a leg/butt man....lol. But as long as they match their clothes right and wear nice styles and all that, they don't need to dress up or look like the s-called Barbie. Barbie looks too fake to me.....I don't like anything fake about a woman.

first part, guys like all types of girls, me personally the crazy fun girls are nice as friends or a 1nighter, but the sweet, nice, shy girls are the ones i wanna take home and possibly marry,
second part, yeah probably initially it would help to dress up to get noticed, but it's definitely not necessary past the initial part of the dating
