no, but you need to let your doctor know you are planning to travel. Especially if you intend to take long flights. If you have a dvt and are taking treatment for it, you probably should not fly at all. That dvt could break loose and cause a stroke.

don't know about a letter but you certainly need to ask your GP for advice.and you need to inform your insurance provider of your condition.
