Generally we don't say it anymore and to be honest the growing feeling in britain is that we should abolish the monarchy and become a republic. Whether that will ever happen remains to be seen.

The US is a republic, while the UK is a constitutional monarchy.

The US does not, and never has had a monarchy. The UK has a long tradition of Royal leadership. In today's UK the monarchy is largely ceremonial in nature but that tradition of "long live the King/Queen" goes back to the time when Kings literally ruled the nation. It is a token of respect to Royalty.

Because the US isn't ruled by the Queen of England. We like Liz and want her to have a long life but we're not about to go shouting around about it.
