you call your bank or go in person during normal hours
you call the credit card company. # on your statement or via online search for the company
they will close the cards out and mail or give you new ones asap.

Cancel the debit card first and immediately. Since it is issued by your bank, call them. If you don't know the number, you should be able to find it on their web site.
Then cancel the credit cards (you have more protection on credit then debit cards, that's why the debit card is top priority). Again, if you don't know the phone number, you can look up the phone number on the credit card company's web site.

One of those helpful tips they always recommend that most of us forget to do is to photocopy both sides of all of your cards and ID so you can cancel easily when lost or stolen (because you still have the info).
Which reminds me I should do mine...but in the meantime, you should be able to get the appropriate phone numbers from recent bills.
