why do they make toilet paper 2 ply and not extra thick?

The Good Housekeeping Institute tested all the leading brands and found that Charmin, Charmin Ultra, Quilted Northern and Cottonelle were particularly slow to dissolve. So if you have a low-flow toilet, or old, sticky plumbing that often acts up, those toilet papers may not be your best bet."

Actually, two ply is stronger than a thicker paper. It's much like the difference between plywood and a single sheet of wood the same thickness. The plywood, by virtue of having several layers of wood, is stronger all in all, than the single sheet of wood.
SO, that is why there is two ply toilet paper. It has a bit more "strength" to hold up until it is immersed in water.

If you make it two ply the space between the plies is just air and doesn't cost them for extra paper pulp. If you weighed a two ply roll and a thick one ply roll you would find the two ply is probably lighter which equals more profit for paper company.
