It's pathetic to admit, but yes, it does occasionally happen.

For example, in 2003 New Mexico State (Div. 1-A) played a game against Division II's Western New Mexico. (At the time, NMSU played in the Sun Belt conference, but now they're in the WAC.) At least they did win 48-3. I'm sure there have been others as well.

And the Division 1-AA schools (now cloyingly known as the "Championship Subdivision") schedule teams in the lower divisions quite often. In fact, every year a couple of them schedule games against Azusa Pacific, which isn't even an NCAA team -- not division 1, II or III. They play in a tiny organization called the NAIA.

To my knowledge, no, but if you dig far enough, you might be able to find one. Suggest looking through the I-A teams at cfbdatawarehouse.com to see if any have played a Division II team in the last 40 years.
