The most common cause of miscarriage is that the embryo didn't form properly. It might be damaged, or geneticly incomplete. It implants, but fails to grow and the body ends up rejecting it. Many women who experienced an early miscarriage, may just assume that they have had a late period.
The second reason is that the embryo isn't properly implanted, or attached outside the uterus. This can be an eptopic pregnancy which is very dangerous for the mother, and almost always fatal to the baby.
Then there is the possibility of the uterus not being able to sustain the growing embryo. This can be due to endometrosus, fibroids, or other gynecological conditions.
Then there are chemical or hormonal imbalances, which kill off the embryo or prevent it forming. Drug abuse can sometimes do that.

I'm not sure I am understanding the question. If you are asking "how" as in, what happens internally, then the answer is the fetus doesn't take hold on to the uterus wall, or is simply rejected, and is released sometimes during "spotting" or during a regular period. Sadly, alot of women have miscarriages every month and don't even know it.
Or...
If you are asking "how" as in what actually causes a miscarriage, its alot of things. Examples would be a traumatic event such as a car wreck, or falling down. Some medications will cause a miscarriage. And some women also have what is referred to as a "weak" or "thin cervix" that will also cause miscarriage later on into the 2nd trimester.

Could be genetic. Could be something with your body chemistry. Could be a disease. Could be the mother suffered some traumatic injury that caused to miscarry.
Pregnancy is actually a type of virus in the eyes of your body. Your body spends most of it's time trying to attack the cells which make a baby...
Some people have bodies that aren't physically able to sustain a pregnancy. Some people say depression can cause you to miscarry. As weird as it sounds, it's not an exact science.

12 weeks pregnant.
