Often people become outsiders because they are used to a certain way of doing/seeing things, and are thrust into a city/school/whatever, where the way things are done are different. When a person decides to be steadfast in their views/ways and don't integrate with the surroundings they find themselves in they become "outsiders." I personally believe this isn't necessarily a conscious choice, but something that just happens.

you are an outsider when anything about you makes it difficult or undesirable to fit in with the norm. since one person can differ from another in countless ways, there are countless factors that can make anyone an outsider, form not sharing the same conversational interests to hobbies to social class to basic personality and outlook on any event or thing.
