Have gradually lost other four speeds over span of two years. Can't find any blown fuses. Are fan speeds from switch , computer or multi-speed motor?

The motor resistor slows the fan down in the lower speeds. It's not used on high speed, so when it goes bad, the fan only works on high.

look at the motor
if only 2 wires come out then it is a single speed motor with a variable resister built in somewhere near the speed switch. remove the switch and on the back of it will be a coil that is probably shot and just letting full power go to the motor. The way it works is power is diverted into a coil along 3 or 4 points. at each point in the spring is a different resistance value. as the lever moves along the way points the amp draw is diminished and the motor runs slower. no way points no resistance, fan blows wide open. Hey its better than no fan at all, and that is the way older models used to do.

I had a cavalier that was the same way, malibu and cavaliers are basically the same. The switch slowly died, one setting at a time. Mine died on speed three first and then one day none of them worked but low. I changed the switch and all of them worked.
