Some old insulation consisted of poly granules which are very flammable, in this scenario it would be necessary to totally remove all the granules, in other words if the insulation burns at all, remove it, if it does'nt burn retain it and put the new insulation on top.

yes, as depending how old the current insulation is, there are so many new products on the market, that I would start fresh, and if you are going to install new insulation, you want the attic as well insulated as possible

Unless the old stuff is wet and moldy......which I'm sure it's not, just put the old stop of top. Read instructions though, you might have to slit something....you doing it yourself? There's one thing you must guard and that's accidently covering any roof vents. You might want to talk to the folks where you buy the insulation if you are doing it yourself and ask about ventilation in the attic. It's critical.

unless it is damage or was was not installed corectly theres no reason to take it out. By putting in blown-in your adding to what you all ready have. Just make sure none of the vents at the low end are blocked off. the more insulation in a roof the better.

Remove the old insulation is probably old and not fire retardant and that old itchy stuff. Go with recycle denim less expensive and a lot healthier.
