That is a fragment. You need to add a verb.
Maybe something like: Jack has a book with a red cover. or I bought a book with a red cover. or The dog stole a book with a red cover.

Fragment. What about the book in the red cover? It's not doing anything, no action is surrounding it, nothing.

sentence fragment because it doesn't make sense...what's with "a book with a red cover"? we don't know...something's missing...so we call it a fragment..it can't stand alone.
"there's a book with a red cover on the table" is a sentence because it makes sense alone.
Good luck!

It is a fragment because it has no verb. A sentence needs both a noun and a verb to be complete.
