I think it should be : PLease take the forms that are sent with this letter to your....
That seems to make sense...

It probably would have been clearer if they had written
"Please take the forms, sent with this letter, to your medical and/or mental health provider to complete."
There were some forms included with the letter that was sent to you (these forms were sent with the letter). You are to take them to your health provider. Your health provider should complete the forms. Then either your health provider or you can send the forms back in the envelope that was provided.

This is the corrected version: Dear Sir / Madam, I am applying for the semester program of IEFE in January 2010 to May 2010. Please kindly find enclosed the registration form and 3 identity photographs. I have also sent you the deposit of the tuition fee (400 euros) through credit transfer.
