The two teams would play a one game playoff. The home team is determined by a coin flip. The game counts as a regular season game, meaning all stats compiled in the game would be credited towards regular season total. And the game counts in the standings, meaning both teams would have played 163 regular season games.

One-game tiebreaker.
If two teams are tied and BOTH are going to the postseason regardless, then non-game tiebreakers are used until the tie is broken. But if two teams are up for one berth, it gets settled on the diamond.
The last tiebreaker game was in 1999 for the NL wildcard. Mets defeated Reds.
The last meaningful tie was in the NL in 2006, when the Padres and Dodgers finished atop the West with 88-74 records. San Diego was awarded the division championship due to a better H2H record (13-5) with Los Angeles, and LA earned the wildcard berth as it had a better record than the second-place teams in the East (Philadelphia, 85-77) and Central (Houston, 82-80).

In case of a tie, the best players from both teams form a new team and go beat Canada, then take turns keeping the trophy in their front offices when they get back. They flip coins to see who is first. but the winner has to buy the loser lots of beer and chips.
If Canada wins, they get to stay free at the Team Managers' houses whenever they're in town for Broadway shows or have Letterman tickets.

I know that their could be a one game playoff between the two teams, but I am not sure if something comes in between the head to head and the one game.

I think the Yanks will, but that is pretty rare. But I honestly think the Yankees will. They have an overall better team.
